Welcome to the new MVLS Interlibrary Loan Request
web form!
This replaces the 4-part
printed form, and may be used to request loans or copies via the MVLS-ILL
department.
Before you fill in the online form:

Interview the patron,
optionally filling out the Patron Interview form (MVLS-ILL has these), or other
form that your library uses for patron ILL requests.
·
Request point-to-point within
CDLC area if you are a point-to-point library, and to the NYSL if you have a
NYSL account. A title that is held locally but does not circulate may not fit
within our guidelines for referral.
How to:

Use a separate form for each
request (for serial requests, for each article). Just fill it out as you would
any web form, click “submit”, and it arrives in our e-mail box.

If you have any other papers
(verification/location info, or what you’ve searched without success) that you
would have attached to the request, please fax or send to MVLS-ILL via
delivery; if you are verifying in a standard source, especially online, and
you’ve given enough information, there is no need to send printouts or copies

Filling out the form: most of this is self-evident, as before

Library: 3-letter code is fine
·
Request for: (choose loan or
photocopy) more detail below.

For monographs loans: fill in Author Last name, First Name, and
Title, Publisher; add medium(s) if not a book, e.g. (DVD videocassette);
Pub date, Price, This Edition only
·
For serials loans: fill in Serial: Title, Volume, Issue, Date;
however, few periodicals circulate from CDLC or other libraries. It is
recommended that serials loan requests be filled out to request copies of pages
or specific information, or even a copy of the table of contents (if so, also Pages
and CCL/CCG). If requesting loan of microform, please so indicate.

Copies: You may request
photocopy from a serial or book, including replacement pages for damaged items.
If the request is for loan of material that is unlikely to circulate
(reference, genealogy), it is recommended that requests be filled out to
request copies of pages, specific information, index entries, or table of
contents.

For photocopies from
books: fill in Author Last name,
First Name, and Title, Publisher; Pub date, Price, This Edition
only; Pages and CCL/CCG; fill in specific edition/ volume for
cited information or replacement pages.
·
For photocopies from
serials: fill in Serial: Title,
Volume, Issue, Date; Article: Author, Title; Pages and CCL/CCG.
·
Pub date: Indicate if a
reprint, and cite original date if known. If date is unknown, please supply any
clues you may have.
·
Price: Unless the patron has
specified a particular edition, give the lowest listed price if the book is in
print.
·
This edition only: It is
assumed that a patron will accept any edition unless stated otherwise. If other criteria exist (e.g., this
translator only), specify in Notes.
·
Copyright compliance
(CCL/CCG): Generally, serials published within the past five years and pages
from monographs in print fall under the fair use Guidelines (CCG) within
limits. Older serials, out-of-print items,
and replacement pages fall under provisions of the law (CCL).
·
Not wanted after: patron
deadline
·
Maxcost ($ willing to pay if
referral out of area needed): while we request from free/low cost libraries
first, we do need to put a $ limit on requests referred out of the region.
·
Verified in: where you found
the bibliographic information or article citation. Note that for serials
photocopy requests both the serial title and the article should be verified.

Open Worldcat (OCLC) is
available to all at www.oclc.org. You can
see holding libraries, but of course, there is no guarantee that they will
lend/copy, or indication of what they will charge (we access the policies
directory for that information).
o
Printed source, give the
author, title, year, volume (if applicable), and page number.
o
If cited online, give URL or
web site (e.g., Amazon); you can copy and paste into the form).
o
Data base verifications:
include the name of the database and article ID number, e.g., Ebsco Masterfile
AN 23097630
o
If not found in any source,
include what you do know, and what was searched. Send a separate note if
needed.

LC#/ISBN/ISSN: at least one of
these is helpful. Match ISBN to the edition that you are requesting.
·
Notes: Any other necessary or
useful information, such as ‘thesis’, or ‘any ed. in Spanish’. Give as much
information as possible if citation is unverified. It can be helpful to know that a book is old, or new. The subject can be helpful if it is not
evident in the title in case the author and title are mis-cited.

Borrower (name or just
initials), Telephone number, Card number; This is for your library's use to
contact the patron and for MVLS-ILL to place hold, and should be treated as
confidential. MVLS-ILL does not include the patron’s name on any requests
referred.
MORE DETAIL
FOR A MONOGRAPH REQUEST:
Author: Last name first, corporate author, or dash
if no author given. An editor or compiler may be indicated by "ed.",
or "comp.", as appropriate.
Title: If the material is not a book, indicate
medium after the title; if patron can use either/any format, say so- e.g., “DVD
or VHS”. Add subtitle, alternate title, or series title if known. Use the Notes
field if more space is needed.
Plays, poems, essays,
short stories: Where a specific play or other short piece is wanted, it is more
important to be specific about what is wanted than to verify a book containing
the piece (e.g. Chekhov, The Seagull, not Chekhov, Three plays), as such pieces
may be found in a collection or anthology.
FOR A SERIALS REQUEST
Serial title: Give the full title if known, as
abbreviations can be confusing. Titles are often abbreviated in indexes and
bibliographies, but there may be a key to abbreviations.
Volume, Issue, Date: At minimum, the volume and year
are included in most citations; give as much information as possible.
Article: if citation for author and title is not
specific, at least give the subject.
Pages: Indicate pages to be copied, table of
contents, or index information. Most libraries limit the number of pages they
will copy, or may charge for exceeding a set number.
